What can we offer?

  • We support the surgeries across the PCN (Bosvena, Lostwithiel, Middleway, Par and Fowey) to complete your annual reviews and follow up appointments.
  • Provide support and guidance to help you live better with diabetes.
  • Monthly support Groups for those living with Type 1 Diabetes and Type 2 Diabetes.
  • Education courses
  • Referrals to other professionals working alongside us who may be able to help you.

What to expect in your annual review

Your annual review should be aimed to be completed in your birth month – if you haven’t had your annual review in a year please contact your surgery to arrange an appointment. During this review we will –

  • Complete your foot check – this will involve talking about foot and nail care, checking your feet, using a doppler to listen to the pulses in your feet and checking the sensation in your feet.
  • Check your height
  • Check your weight
  • Ask you about your smoking status and whether you drink any alcohol.
  • We will do a blood test which looks at the level of glucose in your blood (HbA1C), full blood count, Cholesterol levels and Kidney function.
  • You will be asked to provide a urine sample to also check your kidney function.
  • Check your blood pressure
  • Talk about diet and lifestyle
  • Unfortunately, we cannot perform retinal screening so it is important that you attend the appointments sent to you from the retinal screening team.

It is important that you attend your annual reviews because we know that Diabetes can have severe complications. By having your annual review we can monitor and detect any changes in your health and manage that accordingly.

Once your review has been completed you will be reviewed within 2-4 weeks and made aware of your results and any other concerns. It may be necessary to see you more frequently to help you manage your diabetes better and provide you some more support.

Monthly support group timetable 2026

Our support groups are based in Par at the Cornubia or in Bodmin at Chy Trevail (council offices)

Type 2 support group at the Cornubia in Par is on the first Wednesday of the month between 6-7pm (downstairs).

Type 1 interest group at Chy Trevail in Bodmin is the 3rd Wednesday of the month between 6-7pm.

Type 2 support group at Chy Trevail in Bodmin is the last Wednesday of the month between 6-7pm.

SUPPORT GROUP DATES 2026

Education session timetable for 2026 

We also offer education sessions called Diabetes and You. This consists of three sessions over a course of 3 weeks designed to help you manage and take control of your diabetes. It is a free programme offered to everyone living with Type 2 Diabetes. Please feel free to bring someone with you for support. 

  • Session 1 – Diabetes and You – What diabetes is and how it effects the body.
  • Session 2 – Diabetes, Lifestyle and You – Looks at eating well, activity, sleep, stress, smoking and alcohol intake and understanding what a healthy weight is. Basically all things lifestyle!
  • Session 3 – Diabetes, Food and You – Looks at the different food groups, what impacts your blood sugar levels, diets, and understanding food labelling. Basically all things food!

These sessions are free of any judgement and for you to get out of what you would like from them. We offer these sessions at Chy Trevail in Bodmin and the Cornubia in Par. Please see the link below to view the timetable and if you are interested please contact us and let us know.

Other groups 

  • Walk for health – Based at Lanhydrock on Wednesdays between 11-12pm. Meet at the Cafe. Friends, family and dogs all welcome. Free parking is available.
  • Swimming for Health – Healthy Cornwall alongside Better Leisure offer a free 12 week course of swimming sessions, 6 weeks to improve your ability through the Swim Doctor learn programme and for those who complete the 12 week course of swimming they offer the give it a go 6 week membership. If this is something that you’re interested in please let us know and we can refer you. This is based at Bodmin Leisure Centre.
